Talent Release Form

If you are participating in an event or submitting work to be shared publicly, please fill in a Talent Release form and submit it to IdA.

You can fill and submit this form by using the online form, below. Otherwise, to download and fill the PDF or paper version, you can click this downloadable Talent Release Form, fill it, and email it to office@idainc.org.au or hand it to a staff member. If you need assistance, please call IdA on 08 8948 5400 during office hours (8:30am – 4:30pm Monday to Thursday).

Use this form if your work will be used by Integrated disAbility Action, Inc. (IdA).

IdA wishes to use your photograph, image, words, artwork, video or audio recording.
By submitting this form, you give IdA permission to use any of these items in any way without your further permission.
IdA will, wherever possible, have regard for your cultural, family and personal sensitivities.
